— Ukrainian citizen’s passport
— Identification code (TIN)
— Qualification certificate/confirmation thereof
or specialist certificate for ratings—remember to verify! — Academic diploma with transcript (grades) or a certificate from an educational institution or proof of employment on a ship
— Military registration document (for men aged 18–60)
— 2 recent, matte, 3.5 x 4.5 cm photos (light background)
— Previously issued seaman’s passport (SP) — if exchanging
— Police certificate — if SP is lost (+ translation if necessary)
— Receipt for payment of the administrative fee (original)
